OFFICIAL WAIVER
PLEASE THOROUGHLY READ THROUGH THE INFORMATION BELOW AND AUTHORIZE ELECTRONICALLY SIGN THE FORMS BEFORE YOUR FIRST EVENT
Before participating in a WolfGang, LLC event, you must read, complete, and authorize the general liability waiver as well as the photo/video release forms. These forms will be kept on file until the end of the calendar year. You will be required to fill out a new waiver in the upcoming year.
GENERAL LIABILITY WAIVER
In the exchange for the opportunity to participate in WolfGang, LLC events and activities, I agree and authorize the following:
I understand that participating in an event with a dog requires me to be medically able and I certify that I have no medical or physical conditions that could interfere with my safety in any activity that I choose to participate in with my dog. I am aware and understand that there is always some avoidable risk of injury involved when participating in activities with animals, including risk of injury to myself or my dog. I am willing to assume and bear the costs of all risks that may be created, directly or indirectly by any such condition. I assume all risks associated with participating in WolfGang, LLC events, including but not limited to falls, contacts with other participants/dogs, the effects of weather including high heat and/or humidity, low temperatures, conditions of all the pathways owned by the city or participating locations, and all risks are known and appreciated by me.
I acknowledge the absolute responsibility to follow instructions, guidelines and procedures established by WolfGang, LLC. its employees, and any participating businesses associated with the event or activity. I take full responsibility for my actions and the actions of my dog. I am voluntarily participating in WolfGang, LLC events with the knowledge of any risks involved and acknowledge and agree to assume any and all risks associated with injury, death, and/or property damage, to myself or my dog, caused by negligence or purposefully by anyone participating in the event. I know and understand that WolfGang, LLC events will include various dogs participating and I agree to indemnify and hold harmless WolfGang, LLC and any of it’s employees or agents, as well as any participating businesses for all costs and expenses which may incur due to claims or demands alleging such an injury, and for any injury or damage caused by my dog to any other participant, dogs, public or personal property. I shall indemnify the released parties for any damages awarded or expenses incurred as a result of such claims, rights or causes of action, including but not limited to, settlement payments, attorney fees and court costs, legal fees, and medical fees. I WILL NOT HOLD WOLFGANG, LLC OR ANY PARTICIPATING BUSINESS ACCOUNTABLE OR RESPONSIBLE FOR MYSELF OR MY PETS.
All persons handling a pet must be at least 18 years of age. Any intact male dogs must be closely monitored. Female dogs in heat and puppies under the age of 12 weeks are prohibited from any WolfGang, LLC event. All dogs must remain on a six-foot leash or shorter during the entire event, due to being in public places. NO RETRACTABLE LEASHES ARE ALLOWED! All participants are required to pick up after their dogs and follow city and county ordinances. All dogs must be social with humans and other animals. Any dogs showing signs of fear, stress, or aggression may be asked to leave. NO REFUND PROVIDED! Check your dog’s health and mood before he/she joins us. If your dog is not feeling 100% or seems irritable, please keep him/her at home for the day. You will be showing them just as much love by keeping them resting. I certify and affirm that my pet has been vaccinated, to meet state and local laws. I certify and affirm that my pet is not known to be vicious and it is free of any disease or condition, which would make it a danger to any participant, spectator, or other individuals. I accept all liability and damage to property or persons caused by myself or my pet.
All persons 21 and older are able to drink alcohol freely at their own risk and will be asked to provide photo identification by each participating location. Each establishment is responsible for the amount of alcohol they serve and each consumer is responsible for the amount they consume. This is in conjunction with the Florida State Laws. Any person drinking under the age of 21 assumes the ramifications of such decisions of falsifying their age and is subject to Florida State, Pensacola, and Escambia County laws. Only participants with designated wristbands will be associated with WolfGang, LLC specials at each participating location. Persons who buy tickets for WolfGang, LLC events and consume alcohol do so knowingly at their own discretion, risk and liability. Persons who buy tickets or participate in any event assume all risks associated with drinking, eating, or anything associated in any way with the company providing the drink or food and can not hold WolfGang, LLC liable for injury. Persons who are negligent do so at their own risk and any rudeness or misbehavior will not be tolerated. Any person will be subject, at the discretion of the person in charge, to be removed from any event without reimbursement and law enforcement can be contacted. All participants ASSUME THE RISK for their alcohol consumption and HOLD HARMLESS WOLFGANG, LLC.
THIS IS A WAIVER AND RELEASE OF LIABILITY. I HAVE READ and understand the requirements that this is binding upon me, my spouse, and my children. I confirm that no promises, oral understandings or agreements have been made with regard to the behavior of any dog, the place where they are going/walking, or anything else. By participating in WolfGang, LLC events, I am indicating my voluntary agreement to the provisions of this waiver and release of liability. I understand that this is a contract, which has legal consequences and state that I have read all of its terms and I fully understand them. I am legally competent to execute this release agreement and I accept responsibility for my entry.
BY SIGNING or CHECKING THIS AGREEMENT, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND ALL OF THE TERMS AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE WOLFGANG, LLC.
